Also I feel it must be located some where that is exposed to the air that passes from engine compartment to vents in car because driving with vent on and high heat eventually warms it enough to come on. So was the root cause the relay or both the relay and the fan motor? The one thing I had failed to check the first time was the blower motor relay.

You are doing need to observe instructions and also have loads of persistence however. The blower relay is located on the back of the fuse panel. Sometimes turning it on and off several times will get it to finally blow again. The relay part number is 25230-79942. I swapped the blower motor relay with another and it works great now.
